Work description
Wakefield Council has developed a new model of provision for Housing Related Support, designed around: * Identifying the right services for people the first time * Delivering long term solutions to a person's homelessness, with as few transitions as possible * A Make Every Adult Matter (MEAM) approach - offering coordinated interventions that respond flexibly to meet assessed need * A focus on harm reduction, recovery, resilience and independence; and * Promoting positive approaches to health and wellbeing. A procurement process has already taken place for the Assessment Pathway, Access to Housing Pathway -Main Provision and the Housing Sustainment Pathway. This procurement is for 2 of the Access to Housing -Specialist Provision. The 2 services include; Young Families Service; and Supported Housing for People with Mental Health Needs.
