## Description

Cambridgeshire County Council sought independent consultancy to produce recommendations on how the current Care Together programme might be further evolved into a place-based commissioning and operational model for Adult Social Care \(ASC\) services\. The place-based model will be capable of contributing to the council's strategic ambitions and sustainable management of demand for adult social care\.
