Description
Wiltshire Council, in partnership with Wiltshire Clinical Commissioning Group, is seeking to commission Intermediate Care Beds (ICBs) for Wiltshire. The purpose of ICBs is to support people who meet the eligibility criteria as they are discharged from acute and community hospitals or as a short-term measure as a step-up service from home. ICBs are short term, time restricted, goal-based periods of care that calls on a mixture of health and social care interventions to support people to maximise their potential to live as independently as possible by recovering or continuing to improve skills as a result of those interventions.
