## Description

West Berkshire District Council \(WBC\) wishes to commission a Targeted NHS Health Checks Outreach Service to improve uptake of NHS Health Checks by residents who are under-represented, and disproportionately impacted in terms of cardiovascular health\. This will require highly specialist community engagement, as well as staff trained in delivering NHS Health Checks\. In order to help inform our specification, the Council have drafted a survey which takes 5-10 mins to complete\. Your responses will not be scored as this is an information gathering exercise to understand the market's views on the development of a local model, and the capacity of the market to deliver these services\.
