Work description
Ageing well is a priority area of the NHS Long Term Plan, aiming to help people live well into older age, managing long term conditions with care and technology.
NHS Devon ICB has awarded a contract for a Healthy Ageing service in Torbay to Torbay Community Development Trust following a Request for Quote procedure.
The service includes: • Identification of local individuals most at need of support • Joint approach to managing and supporting person centred interventions to improve life expectancy, reduce the risk of falls and frailty. • Working in partnership with primary care to identify appropriate clients who are at risk of frailty, multi-comorbid long-term conditions and Dementia
The services are healthcare services falling within Schedule 3 to the Public Contracts Regulations 2015 ("the Regulations") which are not subject to the full regime of the Regulations, but are instead governed by the "Light Touch Regime" contained within Chapter 3, Section 7 of the Regulations (Regulations 74 to 77). The contract value of this procurement is under the Light Touch Regime threshold and therefore the rules of this Regime do not apply.
This procurement has been carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
