Post Diagnosis Dementia Advisory Service, Havering Clinical Commissioning Group

The aim of the Post-diagnosis Dementia Advisory Service is to provide non-clinical consistent, highlyresponsive, personalised and emotional support to people in Havering with a diagnosis of dementia and theircarers. The service will provide this through the provision of information to promote self-help; provide awarenessand advice, signpost to other services and offer peer support to dementia patients and their carers. This is toensure service users feel more informed and confident about services available to them.

Additional information: The successful provider is expected to commence mobilisation of the project in April 2019, with the actual project delivery commencing on 01 July 2019.

Bidders must respond to all questions. The deadline for the submission of the tender is 12noon on 21 January 2019.
