Work description
A programme of physical activity in areas with the highest frailty (East Merton Primary Care Network (PCN) and Morden PCN) that will reduce the risk of 'de-conditioning' amongst older people aged 65 and over. The project will run for 17 months from January 2022 and the primary focus will be on people with mild frailty or moderate frailty with the aim to improve their physical condition through physical activity and strength and balance training. The project's secondary focus will be to keep older people without frailty 'fit'. The Provider will be required to work with community and voluntary groups in the area alongside GP practices and other key partners. The provider will use a 'train the trainer' model and there will be small grants programme attached, run by the provider for resident and community groups to run activities.
