Leeds Community Health & Wellbeing Service Delivery Partner

Leeds City Council and Leeds Community Healthcare Trust, working as an Alliance ('the Alliance'), are seeking to appoint a suitably qualified and experienced consultant/ consultancy firm as a Leeds Community Health & Well-being Service Delivery Partner ('Delivery Partner') to support the creation of a new provider partnership between the Council, Leeds Community Healthcare Trust, home care providers and the Third Sector to create the new Community Health and Well-being Service across Leeds. We know the model we wish to implement and intend to trial it first in one of our thirteen Integrated Neighbourhood team areas. Our model will radically change how home care is commissioned, planned and delivered.
