Work description
The Integrated Lifestyle Service Tackling Health Inequalities is intended to be an East Riding wide integrated healthier lifestyle and behaviour change service, which is accessible to all whilst having a strong locality focus and targeting residents in our localities with the largest health inequalities. The service will offer support to East Riding residents to support and encourage them to make positive changes to their lifestyle behaviours to improve their health outcomes and improve their quality of life. The commissioning of this service moves away from the traditional approach of lifestyle services being commissioned to focus on a single issue and will use an integrated holistic approach that takes into account other determinant of health such as personal circumstances, their communities and environment they live in. The service will look to utilise the advances in technology to offer information, advice and support for positive behaviour change and implement the principles of proportionate universalism. Services to be included within the new proposed service are: Lifestyle services (including children and young people) and Smoking Cessation (including stop smoking in pregnancy).
The contract end date (31/03/2028) includes all extension options, the initial end date is 31/03/2026.
