Description

Provision of facilitating community self-care activities that improve anticipatory care, reduce avoidable crisis and support joined up care health services to patients registered with GPs in West London. The service delivers essential capacity to the local Whole Systems Integrated Care Pathway currently under development. The interim contract will ensure continuity of essential service provision of Whole Systems Self-Care Support Service, to the care of over 65s population, whilst a new integrated model of care is developed during the contract period. This notice refers to a Direct Award of an interim contract to an incumbent service provider to ensure continuity of essential health services to the local population whilst a new integrated care model is developed during 2018/19 ready for procurement in 2019/20 and therefore the CCG has assessed that the incumbent provider is the only capable provider at this time. A 1 year contract has been awarded, commencing 1st April 2018, with the option of an additional 1 year extension at the CCG's discretion. The Contracting Authority is developing a new model of care for the over 65s within West London which will establish integrated pathways in regard to physical health, social care and mental health, enhanced with voluntary sector involvement to deliver holistic patient care. The new My Care My Team model requires the existing expertise and skill set of the Whole Systems Community Self-Care Support Service to form an integrated core team approach to delivering care. The current interim contract is necessary to enable NHS West London CCG to develop, pilot and validate its specified service requirements. The CCG plans to engage with the market during 2018 to assist development of its future specification of requirements, with the intention of informing the process to award substantive contracts to deliver the new model of care from 2019. Market engagement will commence March 2018. Additional information: This interim contract is being awarded to the incumbent provider to ensure continuity of essential health services to the local population whilst the CCG develops and substantive new service with increased scope and value. The direct award without a competition is to the only capable provider having the capability and infrastructure in place to enable the Contracting Authority to develop and test out its new service model.
