GB-London: Better Care Fund Support Programme 2023-2025 - York - Discharge Planning Support and Pathway 3 Review

The buyer is seeking a contractor to provide support to York Place, to drive the planning and implementation of improvements identified in a recently-undertaken discharge stocktake. Additionally, the system would like to review its Pathway 3 policy and process - looking across the whole patient journey, with a view to improving long term outcomes for people leaving hospital on this pathway.

Additional information: The contracting authority considers that this contract may be suitable for economic operators that are small or medium enterprises (SMEs). However, any selection of tenderers will be based solely on the criteria set out for the procurement.
