Work description
Care and Support at Home Mobile Night Service to operate overnight (21:00 - 06:00) 365 days of the year. The service will deliver both planned and unplanned care, working seamlessly and collaboratively with other services to support people in their own homes, preventing admissions to acute care settings, reducing length of stay in hospital and facilitating hospital discharge
