ODN - London Cardiac Rehabilitation App

We are seeking to procure a digital tool (either an application OR website - preferably a tool that can work on either) to aid delivery of a Cardiac Rehabilitation (CR) programme. This tool needs to support the below core features: • A Physical Rehab / Exercise programme - both pre-record and ability to host live classes • An Education programme - a combination of text, video and hyperlinks • The functionality for the patient to self-monitor from home: Blood Pressure, Pedometer, Weight, Exercise/food diary, goal setting etc... • Communication between patient and clinician
