Population Health Management (PHM) digital solution

Description

The Norfolk and Waveney NHS Integrated Care Board and its partner stakeholders have a requirement for a digital solution to support their programme of designing and delivering improvements to population health and wellbeing outcomes, now and for the future, which will be flexible over time as requirements and capabilities change. The digital solution must provide bespoke data analysis, technical solutions and digital platforms to underpin our PHM strategy and support the prioritisation and delivery of interventions. Delivery of interventions at system level requires the ability to risk stratify cohorts of population in line with N&W's identified priorities and opportunities N&W ICB wish to enable access to and use PHM information at all levels, across NHS and wider partners within the Integrated Care System (ICS) to support a strategically aligned approach to Population Health.
