Support at Home Service - East Riding of Yorkshire

Description

The service will provide a flexible range of provision in an individual's own home with both a proactive and responsive approach, offering practical and emotional support at home in response to escalating needs (admission avoidance) and rapid same day support following discharge from an acute hospital bed (early supported discharge) within the East Riding of Yorkshire area. The service works in collaboration with health and social care partners to deliver a coherent and coordinated plan of care and support improvements in care pathways in East Riding of Yorkshire.
