NHS South West London ICB- The Provision of a Case Management Tool

Description

The procurement is for a case-management system to support delivery of the Proactive Anticipatory Care (PAC) programme in Kingston and Richmond places. PAC is an MDT model of proactive care which aims to target people who have escalating health and/or social care risks. The programme aims to keep people more independent and healthier at home for longer. To support the delivery of PAC, a case-management system is required to store care plans and monitor actions in a place that is accessible to partners from multiple agencies. The duration of the contract under the G-Cloud 14 call off terms and conditions will be twelve (12) months.
