## Description

The 'Moving On Up Project' aims to support people with Complex Needs towards self-management of their own condition\(s\), develop independence/daily living skills, integrate into the community, develop meaningful relationships, navigate services and \(where appropriate\) manage a tenancy\. The service is primarily intended for young people \(aged 16-25\) with Autistic Spectrum Conditions and/or Attention Deficit Hyperactivity Disorder\. It incorporates a 24 hour Supported Living service, In-reach support, Outreach support, Self-management support and Crisis Management\.
