The Department of Health and Social Care is seeking to understand current good practice in how co-ordinated support can be provided to people with a learning disability to ensure their health and wellbeing. The procurement aims to select a provider who can undertake a short exercise in identifying good practice and innovation in providing co-ordination or key working support for people with learning disability in accessing health and care services, and living healthily.
