## Description

The City of York Council of the City of York and the NHS Humber & North Yorkshire Integrated Care Board \(the "Commissioners"\) are looking to commission up to 10 Discharge to Assess \(D2A\) Beds on a block purchase basis across a range of care home facilities to support our patients and customers following discharge from hospital on discharge Pathway 2\.
