## Description

The Patient Discharge Service will provide early supported discharge services to a cohort of patients who no longer require secondary care within the hospital and need to transfer to another setting to meet their ongoing needs, such as nursing, residential or re enablement facility or a package of support within their own home\. This tender is seeking a Sub-Contractor to provide early supported discharge services which provides brdging packages of care visits, to enable patients who are medically safe for transfer and no longer needs to be in a secondary care setting of our hospitals for their ongoing care\. The contract duration will run from May 2023 until 31st March 2024, and there after subject to funding availability\.
