Transfer Of Care from the Community Reablement Service Rounds - AWARD

The Council is seeking tenders from Service Providers currently contracted under the Framework Agreement for the Provision of Care and Support at Home Services (2015), the Domiciliary Care Framework Agreement (2009) and the Care and Support at Home Contingency Services Agreement (2021) to deliver a round of domiciliary care services to support the transfer of care services from the Community Reablement Service.
