Work description
This is a time limited and highly focused 12-week intervention which involves one to one community support, to support citizens to recover and help to overcome issues in their daily lives that impact negatively on their mental health and wellbeing. The primary focus of the service is to enable citizens to remain independent in the community and to facilitate discharge from psychiatric wards.
