Support, Information and Advice Service for Dementia

The Council's requirement under this Contract is the delivery of a service that will provide support, information, and advice for people upon diagnosis of Dementia. This service will also provide support for informal carer(s) and family. Further information is provided in the specification.

The contract is for a period of three years with the option to extend by a further one year.
