## Description

Deliver an improved digital service for Healthcare Professionals to submit an SR1 medical evidence form and help end of life patients get the support they need under Special Rules for End of Life \(SREL\)\. In addition to delivering financial support, this work will improve timely processing, provide modern and flexible services for our citizens and help to reduce fraud and error\.
