Work description
(1) Community @home service: provision of dedicated GP, driver and vehicle to support Urgent Community Response provision, in the home setting. (2) Acute GP hub: provision of x1 GP at St Thomas' hospital and x2 GPs at Guy's Urgent Care Centre.
