Supported Living or Outreach urgently required for client on the attached form.
Location: Cheshire West and surrounding areas
A successful provider will be invited to enrol in the Council's Dynamic Purchasing System for LD Autism and Mental Health which is the usual route for commissioning these services. The DPS contract is attached for reference.
