End of Life Domiciliary Services & Care Coordination Service in Northamptonshire

NHS Nene and NHS Corby Clinical Commissioning Groups (the "CCGs") are seeking Expressions of Interest from suitable Providers who are interested in being the provider of a 12 month caretaker contract for the following service(s): Service • An End of Life Care Coordination Centre (including User Call Centre Support) • End of Life Rapid Response Service including o Rapid Response Services provided by qualified End of Life trained nurses o Patient and carer support provided by activities of daily living workers o Primary Care End of Life Link Nurses based in the acute hospitals The CCGs are currently considering this as a single 'lot'. The contract period will be 12 months and the intention is to establish a local evidence base for an improved pathway and identify the gains and any possible innovations to the End of Life Service which can be achieved from this approach delivering the best possible outcomes to patients.
