Work description
Adults' Health and Care sought a provider that had accommodation to develop a tiered supported housing service based in north east Hampshire for working age adults who experience severe and enduring mental health challenges including young people coming through transition (18+). The service will also support people with secondary needs for example learning disability, physical disability, substance use issues and autistic spectrum conditions. The supported housing service will encompass both Younger Adults Extra Care Housing (YAECH) and Stage 1 and 2 plus services across the whole site. The key aim is to provide facilities and flexible associated support and care which enables individuals to manage their mental health in a recovery focused way whilst remaining as independent as possible in their accommodation.
