Work description

The Leeds health and care system ('the Leeds system') is looking for a contractor to survey a sample of the adult population of Leeds with two outcome tools, analyse the data collected and present the findings.

The contractor will be required to use the PROMIS-GHS and P3C-EQ tools to assess how adults in Leeds feel about the quality of the care they receive and how person centered it is. The results of this should be presented as representative of all local care partnerships in Leeds (see below for details), covering three populations (healthy, those living with long term conditions and living with frailty), and include analyses by deprivation, age, ethnicity and gender.

The learning from this will help us evaluate the different local care partnerships in the City, how effectively they support their populations and if they are addressing the issues of population health outcomes and health inequalities in the City effectively.

The learning will be used to inform the development of services in Leeds across the health and care sector. These results will be used to create a baseline and separate surveys will be carried out in subsequent years to measure progress in improving population health outcomes and reducing health inequalities.
